All scholars must complete a brief entry survey in addition to the paperwork requested by your school of nursing.

Take a moment to complete your survey! Once your survey is completed we will send a welcome packet to your School of Nursing.

Scholars who began their program in Round 4 of funding, or August 2011 – August 2012, should navigate here to complete their entry survey.
